Comme pour Illmatic de Nas, sorti plus tôt la même année, Biggie fait le choix d'une pochette qui le représente enfant. Un mini-sosie arborant une coupe afro, et qui illustre parfaitement le concept de son premier album : la vie de l'artiste depuis sa naissance jusqu'à sa mort, inévitable.
