Le colonel Ethan Willis rejoint l'équipe d'Angels Memorial et embarque le Dr Mike Leighton avec lui pour venir en aide à des victimes d'attaque de requins à Malibu.

Un bus scolaire et un camion se sont rentrés dedans. Plus de 15 victimes sont déjà dénombrés avant même que le compte final ne soit fait. Mike est dans le coma après la chute. Angus espère que son frère se réveille vite.

Après avoir suivi les instructions de son livre de médecine et ayant fait perdre sa jambe à un sportif. Charlotte n'a plus le droit de pratiquer sans être sous la supervision d'un médecin. Dans les sous-sol, un des employés de l'hôpital est coincé sous des débris. Seule Noa est assez fine pour l'atteindre. Mario lui dit comment faire pour arrêter l'hémorragie.

Halloween. Campbell préconise une trachéotomie pour gagner un peu de temps et le père d'Angus, fidèle à lui-même, fait douter Angus dans ses positions. Amanda Nolan a emmené le fils de son petit-ami dans un théâtre connu pour une série de meurtres. Elle tombe sur Ethan avant de partir. Ils apprennent qu'il y a eu un accident au théâtre et Ethan demande l'autorisation de prendre deux autres médecins pour aider sur place.

A code black at Angels Memorial takes a turn for the worse when a city- wide blackout leaves the hospital without power or back-up generators. As Campbell and Leanne divert high-risk patients to a nearby care center, Elliot and Heather are trapped in an elevator with a woman in labor, and Willis and Dr. Nolan try to help a 92-year-old woman who thinks it’s 1942.
